What these forms collect

Evaluation registration collects the athlete’s name and birth year, a parent/guardian contact name, email, phone number, selected session, booking status and the time and version of your acknowledgment. The full date of birth is used to check age eligibility when you submit; only the birth year is retained in the booking record. Team inquiries collect your name, email, organization and message.

How the information is used

These details are used to organize evaluations, manage spaces and waitlists, respond to inquiries and contact you about your request. Submitting a form does not subscribe you to marketing.

Storage and access

Submissions are stored in the website’s hosted database using ChatGPT Sites and Cloudflare infrastructure. They are not displayed publicly. RISE’s authorized site operator can access records to administer bookings and inquiries. Do not submit medical records, payment details or other sensitive information in the team message field.

Athlete portal

The portal uses an email verification code to identify your account. Supabase verifies your email and manages sign-in sessions; Resend delivers your one-time code. Essential secure cookies keep you signed in. Existing users can still use ChatGPT sign-in to connect their existing profiles to a verified email account. Connecting accounts requires verification of both sign-in methods. It stores athlete names, age groups, positions, development goals, your acknowledgment and coach evaluations, including ratings, observations and development recommendations. Profiles are private to the account that creates them and authorized RISE coaches. The Athlete Ownership section also stores athlete reflections, coach observations and published development reviews. Reflections are visible to the profile’s account and authorized RISE coaches. Coach observations remain private to coaches until selected for a published review; a published review shares the selected notes, counts, context and ratings with the profile’s account. Coaches can publish evaluations and review development history. Your account identifier links the profiles to you; coach access is assigned by RISE and is not granted by signing up for an email account. Athletes under 13 must use a parent or guardian’s account. Avoid medical information and other unnecessary sensitive details in profile goals, reflections or coach notes. Contact RISE to correct or delete a profile, evaluation or ownership record.

Your choices

You may contact RISE to change or cancel a booking, withdraw your request or request deletion. Some information may need to be retained for applicable recordkeeping obligations. For minors, a parent or guardian should complete the form.

Participation and email

This notice is not a participation waiver. Any required participation documents must be provided and completed before activity. The forms display a saved confirmation on this website; the confirmation screen tells you whether an email copy was submitted for delivery. When enabled, Resend processes the recipient email address and confirmation content to send transactional receipts.
